Dr. Elettra Fiengo
Lipoedema is a multifactorial, underdiagnosed condition characterised by a disruption of the connective tissue, systemic inflammation, difficulties in the reabsorption of extracellular fluids, adipocyte hyperproliferation, fibrosis, permeability, and metabolic, hormonal, immune, and vascular alterations.
The symptoms can impair daily activities from the early stages, eventually leading to compromised mobility. For years, treatment proposals have focused on managing vascular problems; however, there is now sufficient evidence of involvement, through the connective tissue, of the articular system, myofascial system, peripheral nervous system, digestive system, and potentially many other body regions.
The physiotherapist’s role is to raise awareness, restore the lost skills to the patient, open new possibilities for life, and educate on the management of a chronic condition, using all available tools and techniques.
In both theory and practical sessions, this course will give participants the opportunity to approach a new method of physiotherapy assessment and manual treatment, developed from experience gained in the treatment of over 400 lipoedema patients.
Together, we will delve into the heart of the guidelines to face the challenges posed by this condition, which is still relatively unknown, with a critical and constructive attitude.
We will learn to integrate the available rehabilitative techniques to provide patients with the necessary tools to manage difficulties effectively.

PHYSIOTHERAPY ASSESSMENT
• Medical history and family history: age, onset, trauma, diseases and surgeries, diet, physical activity, comorbidities
• Signs and symptoms: pain, joint issues, vascular implications, motor difficulties
• Visual observation and palpation: posture, volume, presence of oedema, vascular alterations.
• Evaluation of compartments and fascial chains, assessment of fibrosis from a functional perspective.
• Hypermobility, hyperlaxity, and joint movement deficits or fascial gliding issues: strength, activation, motor control, balance, motor and connective competence: speed, fluidity of movement, movement across the three planes of space.
• Strategy: where to start and how to set up treatment
• Pain: heaviness, spontaneous pain, tenderness, hypersensitivity, joint pain
• Stiffness, fascial compartments, restoring mobility: mobilisation, manipulation, active and passive mobility
• Oedema: superficial and deep drainage difficulties, how it changes, reduction through exercise
• Fibrosis treatment: manual therapy and with tools
• Precautions and post-treatment side effects, duration and dosage
• Therapeutic exercise, physical exercise, self-treatment
• Post-surgical patient, complications: post-surgical fibrosis, oedema in other regions, weaning off compression, pain
